Canyon Home Care expands services with Columbine Poudre acquisition

Canyon Home Care & Hospice has taken over Columbine Poudre Home Care and Bloom at Home. The move aims to improve care services across Northern Colorado, ensuring patients face no disruptions during the change.

The acquisition will expand Canyon's range of support, from medical and non-medical home care to hospice and infusion services. The transition for existing patients will be smooth, with care teams, treatment plans, and scheduled visits staying the same. Employees from Columbine Home Care have been offered roles within Canyon to maintain continuity for families.

In 2025 alone, Columbine Home Care provided over 10,000 visits, while Canyon delivered more than 49,000. This highlights the growing demand for home-based care in the region.

Holly Johnson, formerly of Columbine Home Care, has now joined Canyon. She described the merger as a natural fit, strengthening the full range of care available. The change is mostly administrative but will improve operations and support for patients.

Canyon serves communities from Wellington to Longmont, including Fort Collins, Loveland, Greeley, and parts of Denver. The acquisition will help meet the increasing need for reliable home care and hospice services in these areas. The takeover will not alter day-to-day care for patients but will broaden Canyon's ability to deliver essential services. With a larger team and expanded resources, the organisation is set to provide stronger support across Northern Colorado.
